Live Well Boone County

Live Well Boone County is a collaboration of organizations that engage in a community-wide strategic planning process. This process results in the Boone County community health assessment and improvement plans. The process helps gather data, prioritize public health issues, identify resources to address those issues, engage the community, and take action to improve conditions that support healthy living throughout the county. The current strategic issues are barriers to healthcare, health behaviors, and mental health. Each strategic issue has an action team. These teams have identified county-level health indicators. They have also developed strategies and activities to address these indicators.  View the Live Well Boone County Reports here: https://www.como.gov/health/about-us/reports/
